To solve the problem of old-age care, it is urgent to improve the social security system as soon as possible, and to accelerate the improvement of the overall level of national social security, especially the unification of old-age insurance and medical insurance, is particularly important." Nie Pengju believes that at this stage, the contribution rates of old-age insurance and medical insurance are not uniform across the country, and there are also great differences in the social insurance payment base. This difference not only makes it difficult to calculate national social security pooling, but also makes the transfer procedure of social security relationship between different places complicated and less convenient. It also exacerbates the imbalance of old-age care costs between urban and rural areas, and between eastern and western regions, and lacked fairness.

"In the long run, it will seriously affect the mutual assistance functions of the old-age system and the medical insurance system, and profoundly affect the labor cost structure and labor flow under the market economy, which is not conducive to promoting regional and urban-rural coordinated development." Nie Pengju said that the specific adverse effects can be from three perspectives: First, due to the insufficient level of social security co-ordination, the flow of talent is restricted; Second, the proportion of social security payment and the base of social security payment in provinces, autonomous regions, and municipalities across the country are not uniform, which is not conducive to national coordination and cross-provincial settlement; Third, pension insurance and the proportion of medical insurance units paid into personal accounts is different in different provinces, autonomous regions, and municipalities. This difference has contributed to differences in population structure in different regions and the resulting differences in enterprise payment burdens, as well as an unfair environment for regional economic development and competition, which is not conducive to industrial upgrading and the formation of new competitive advantages in the eastern coastal areas, are also not conducive to the transfer of traditional manufacturing industries from the eastern region to the central and western regions and the development of labor-intensive industries in the central and western regions.

"The national unified endowment insurance and medical insurance contribution ratio should be designed at the top level and promoted from top to bottom, so that it can be realized slowly." Nie Pengju suggested that the goal could be reached gradually from several areas.

Gradually realize the national social security co-ordination. Due to different economic development levels, government policies, and service systems among provinces, autonomous regions, and municipalities across the country, achieving national social security co-ordination is not necessarily a one-time matter. It requires mutual support and coordination among provinces, autonomous regions, and municipalities, and adjustment of implementation plans. Only in time can we finally determine the trial time and reasonable rate of the new social security premium rate, and gradually realize the national social security co-ordination.

Improve the inter-provincial settlement system of social pooling funds. At present, China has basically realized the inter-provincial settlement of social security co-ordination funds, but the degree of realization is different, there are great differences between provinces, autonomous regions and municipalities, there are also many problems in the co-ordination area, and there are many restrictions on cross-provincial settlement. Social security card or medical insurance card can only achieve inter-provincial settlement in designated hospitals, so it is necessary to further improve the inter-provincial settlement system of social pooling funds and establish a more convenient and effective social security system.

Deepen the reform of the medical security system and establish a unified national basic medical insurance rate mechanism." This can be achieved by accelerating the reform of the medical service supply, establishing a market-led pricing mechanism for pharmaceuticals and medical consumables, and establishing a national transaction price information sharing mechanism; optimizing the medical security public management services, by establishing a unified, efficient, compatible, convenient and Secure medical security information system, realize the interconnection of national medical security information, strengthen the orderly sharing of data, and gradually establish a more complete basic medical security system covering all people to deepen the reform of the medical security system." Nie Pengju  said.
